More About 1:4:2 Surgical Contra-Angle Handpieces

Kaneiko 1:4:2 surgical contra-angle handpieces are speed-increasing electric dental handpieces designed for controlled high-speed cutting during compatible oral surgery procedures. Their 45° angled head improves access to posterior and restricted treatment areas where a conventional contra-angle head may limit visibility or bur positioning.

The 1:4:2 speed-increasing transmission works with a compatible electric micromotor and provides a manufacturer-specified maximum rotation of up to 168,000 min⁻¹. This allows suitable FG friction-grip burs to be used for tooth sectioning, root separation, hard-tissue cutting and other compatible surgical procedures requiring controlled access.

The collection includes the Kaneiko 1:4:2 45° Surgical Speed-Increasing Electric Contra-Angle Handpiece for FG Burs in two water-delivery configurations. Model E45L features three-point internal water spray for chairside and surgical use, while Model E45L-X features single-point external water spray for compatible motor and MOS use.

Both models feature fibre-optic light transmission, ceramic bearings, a stainless steel body, Clean Head System and Anti-Heat System. They accept FG dental burs with a standard Ø1.6 mm shank.

Introducing 1:4:2 Surgical Contra-Angle Handpieces in Dentistry

Surgical access in posterior areas can be restricted by adjacent teeth, soft tissue, limited oral opening and the angle of approach. A 45° surgical contra-angle helps position the bur more directly toward the operative area while allowing the clinician to maintain a comfortable hand position.

Unlike a conventional air-turbine surgical handpiece, this collection is powered by a compatible electric micromotor. The electric drive system supports controlled rotational performance, while the speed-increasing transmission provides the high operating speed required for suitable surgical FG burs.

The angled head is especially useful where a standard contra-angle body may interfere with visibility or instrument positioning. It can support improved access during compatible molar sectioning, root separation and hard-tissue cutting procedures.

Clinical performance depends on more than maximum speed. Bur selection, water delivery, fibre-optic illumination, bearing stability, motor compatibility and correct operating technique all affect cutting control and procedural safety.

What Are 1:4:2 Surgical Contra-Angle Handpieces Used For?

Posterior Oral Surgery Access
The 45° head supports improved positioning in posterior areas where a conventional handpiece head may restrict access.

Model E45L with Three-Point Internal Water Spray

Model E45L features a three-point internal water spray system designed to distribute cooling water around the FG bur through channels inside the contra-angle handpiece.

The multi-point internal spray supports cooling, debris removal and visibility during compatible surgical cutting procedures. Its integrated water pathway also provides a cleaner setup without requiring an external spray pipe.

Model E45L is intended for chairside and surgical use where internal water delivery is supported by the compatible electric micromotor system.

Model E45L-X with Single-Point External Water Spray

Model E45L-X features a single-point external spray pipe that directs cooling water toward the FG bur and operative area.

This configuration is designed for compatible motor and MOS setups where external water delivery is required. The position of the spray pipe should be checked before operation to confirm that cooling water reaches the working end of the bur.

Model E45L-X retains the same 45° head, 1:4:2 transmission, fibre-optic illumination, ceramic bearing and stainless steel construction as Model E45L.

45° Angled Surgical Head

The 45° head is designed to improve bur access in posterior and restricted areas. Compared with a conventional contra-angle head, the angled configuration can reduce interference from adjacent teeth and oral structures.

Improved access can support more controlled bur positioning during tooth sectioning, root separation and compatible impacted-tooth procedures. The clinician should still select the bur shape and length according to the operative area and manufacturer instructions.

The 45° design is procedure-specific. It should not automatically replace a standard restorative contra-angle or a dedicated implant reduction handpiece.

1:4:2 Speed-Increasing Transmission

The 1:4:2 speed-increasing transmission provides controlled high-speed bur rotation when connected to a compatible electric micromotor.

The manufacturer-specified maximum rotation is up to 168,000 min⁻¹. The motor settings should be configured according to the handpiece instructions and the selected surgical procedure.

Do not interpret the official 1:4:2 notation as a different decimal gear ratio unless separately confirmed by the manufacturer. The official product notation should remain written as 1:4:2.

  • Review cleaning, lubrication and sterilisation requirements

The correct model should be selected according to the electric micromotor, water-delivery configuration and intended surgical procedure rather than appearance alone.

How to Choose the Right 1:4:2 Surgical Contra-Angle Handpiece

Choose E45L for Internal Water Delivery
Select Model E45L when the compatible electric system supports internal irrigation through the handpiece.

Choose E45L-X for External Water Delivery
Select Model E45L-X when the motor or MOS setup requires a single-point external spray pipe.

Confirm the Surgical Procedure
This collection is intended for compatible posterior oral surgery, tooth sectioning, root separation and hard-tissue cutting procedures.

Confirm Motor Compatibility
Check the connection, speed range, fibre-optic system and water-delivery configuration before purchasing.

Select the Correct FG Bur
Use an FG surgical bur with a Ø1.6 mm shank that is approved for the selected operating speed and clinical procedure.

Do Not Substitute It for an Implant Contra-Angle
Implant drilling and placement normally require a dedicated 20:1 speed-reducing contra-angle and an implant motor with appropriate torque control.

FG Bur Compatibility

Model E45L and Model E45L-X accept FG friction-grip dental burs with a standard Ø1.6 mm shank.

The selected FG bur should be suitable for surgical use, the intended operating speed and the required access depth. Bur shape, cutting design and working length should be selected according to the surgical procedure.

FG burs should not be confused with Ø2.35 mm RA latch-type burs used in standard low-speed contra-angle handpieces or HP burs used in straight handpieces.

To understand the differences between rotary-instrument systems, read the FG, RA and HP Bur Compatibility Guide for Dental Handpieces .

Always inspect the bur before use. Bent, worn or incorrectly fitted burs may increase vibration, reduce cutting control and place unnecessary stress on the chuck and bearing system.

Internal vs External Water Spray

Model E45L uses three-point internal water spray delivered through the handpiece. Water is distributed around the bur from multiple directions to support cooling and removal of cutting debris.

Model E45L-X uses a single-point external spray pipe. This allows irrigation to be directed toward the bur and treatment area from outside the handpiece body.

The selected water system must match the connected motor and dental-unit configuration. Before treatment, confirm that water reaches the working end of the bur.

Fibre-Optic Light Transmission

Both models feature fibre-optic light transmission. Illumination is carried from a compatible electric micromotor through the handpiece toward the operative area.

Improved visibility can support accurate bur positioning during posterior and restricted surgical procedures. The connected micromotor must support fibre-optic transmission for the light feature to function.

Ceramic Bearing and Stainless Steel Body

Ceramic bearings support smooth and stable rotation at high operating speeds. Stable bearing movement can help reduce vibration and improve bur control during compatible surgical cutting.

Both Model E45L and Model E45L-X feature a stainless steel body designed for durability and stable clinical handling. The smooth exterior supports routine cleaning and sterilisation according to the recommended instructions.

Clean Head and Anti-Heat Systems

The Clean Head System helps reduce the entry of treatment debris into the handpiece head.

The Anti-Heat System supports temperature management during operation. These protective features do not replace correct water cooling, operating technique or regular handpiece maintenance.

The handpiece should be cleaned, lubricated and sterilised according to the product instructions after clinical use.

Electric Micromotor Compatibility

Kaneiko 1:4:2 surgical contra-angle handpieces are designed for use with compatible electric micromotors.

Before ordering, verify the physical connection, maximum speed, fibre-optic capability and water-delivery system. Model E45L requires a compatible internal water configuration, while Model E45L-X uses external irrigation.

The motor settings should be configured according to the handpiece specifications and the selected surgical procedure.

Maintenance and Sterilisation

Proper maintenance supports gear transmission, bearing stability, water spray performance and secure bur retention. After use, remove the FG bur and clean the exterior according to the manufacturer instructions.

Flush the internal or external water pathway and inspect each spray outlet for blockage. Lubricate the handpiece through the correct maintenance connection when required.

Increased noise, vibration, heat, reduced rotation or loose bur retention may indicate that cleaning, lubrication or professional servicing is required.

Read the Kaneiko Dental Handpiece Maintenance Guide for additional cleaning, lubrication, inspection, sterilisation and storage guidance.

Professional Insight

In clinical practice, a 45° surgical contra-angle is selected primarily for access. The angled head can improve bur positioning in posterior areas where a conventional head and handpiece body may interfere with the operative approach.

Model selection should begin with the irrigation system. E45L provides integrated three-point internal spray, while E45L-X provides an external spray pipe for compatible motor and MOS configurations.

The handpiece should not be selected based on speed alone. Bur compatibility, water delivery, motor connection, fibre-optic illumination and the required surgical access are equally important.

A worn or incorrectly selected bur can reduce cutting efficiency and increase vibration. Adequate cooling and light pressure should be maintained according to the procedure and bur manufacturer instructions.
